Fjör Integrated Wellness ("Fjör", "we", "us") provides nurse-led infusion services at Suite 12, 200 Malop Street, Geelong VIC 3220. As a health service provider we are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles. This policy explains what personal information we collect, why, and how we handle it.
What we collect
- Contact enquiries: when you use the form on this website or contact us directly, we collect your name, phone number, email address, the service you are asking about, and your message.
- Booking and clinical information: when you book or receive care, we collect the information needed to treat you safely, including identity and contact details, date of birth, relevant health history, allergies, medications, your prescription or practitioner authorisation, and records of the care we provide. This is health information and we treat it with particular care.
- Payment information: payments are processed by our payment providers. We do not store your card details.
Why we collect it
We collect personal information to respond to your enquiries, to assess your suitability for treatment, to provide care safely, to communicate with you about appointments and payment, to correspond with your referring or treating doctor, and to meet our legal and professional record keeping obligations.
Who we share it with
- Your treating practitioners: we send a summary of your care to your referring doctor, and may contact your prescriber about your treatment.
- Service providers that help us run the clinic: Halaxy (practice management and clinical records, based in Australia), FormSubmit (relays website form messages to our email and may process data overseas, including in the United States), our payment providers, and Cloudflare (website hosting). We take reasonable steps to deal with providers that protect your information.
- Where the law requires or permits it, for example mandatory reporting or responding to lawful requests.
We do not sell personal information, and we do not use it for advertising profiles.
Website data
This website does not use advertising trackers or analytics cookies. If you choose not to use the contact form, you can always reach us by phone or email instead.
Storage and security
Clinical records are held in our practice management system with access limited to the people who need them to provide your care. We take reasonable steps to protect personal information from misuse, loss, and unauthorised access, and we keep health records for the periods required by law.
Access, correction and complaints
You can ask for access to the personal information we hold about you, or ask us to correct it, by contacting us at admin@fjor.com.au or 0493 016 456. If you have a concern about how we have handled your information, please contact us first and we will do our best to resolve it. You can also complain to the Office of the Australian Information Commissioner at oaic.gov.au.
